City of Kelowna / Kelowna Airport

Manulife is the benefits provider.  Please see  ManuLife website.  The plan contract number is 0077313.  The member certificate number is your employee number followed by 4 zeros.  You will need to register and set up a password.  Take a look atthe City of Kelowna Benefit Information Booklet or download a Manulife Extended Health Care Claim Form & Dental Benefits Claim Form.

District of Lake Country

Sunlife is the benefit provider.  Please see in-house website for forms & more information.

Glenmore Ellison Improvement District

Group Source is the benefit provider.  Please see Claimsecure website. You will need to have an Active Benefit Card and email address to register, if you aren’t already registered.  Take a look at the  or download an Extended Health Claim or Dental Claim.

Regional District of the Central Okanagan

Canada Life is the benefit provider.  Please see CanadaLife website .You will need to have Canada Life Benefit ID Card to register, if you aren’t already registered.  Take a look at the Benefit Overview or download a Health Claim Form, a Drug Claim Form, a Vision Claim Form or a Dental Claim Form.
